The project created a prototype for a web-based tool to support local, state, and federal home visiting stakeholders’ ability to better understand and coordinate community referrals. The project used a human-centered design framework to deeply understand stakeholder needs and develop a tool that is responsive to those needs. Project activities included stakeholder engagement; a scan of relevant tools, methodologies, and technologies that could inform the proposed tool; the development of a logic model; exploration of a variety of data sources; development of analyses to feed the tool; and an iterative prototyping process.
